Online registration will open July 1st for fall season.

 

All paperwork will be collected during one of our onsite registrations. Not having the proper Paperwork will keep your child from participating, so it is imperative that we have the following so that your child is eligible.

 

 

• Contract and Parental Consent Form*

• Covid waiver*

• Physical Fitness and Medical History Form*

• Updated Physical*

• Parent and Player Code of Ethics*

• Parent/Coach Communication Plan*

• Copy of the Player's Birth Certificate*#

• Player must have state ID*

REGISTRATION FEE: Fee is 150$ to play. The fee includes use of helmet, shoulder pads, gameday pants and jersey. Players will be required to provide cleats and mouthpiece.

 

AGE: July 31st is the cutoff date for age change. So, you can play with your designated age group if your birthday is after July 31st.
